Required Documents:

1. Complete one copy of the Final Examination Form for the Undergraduate Program
2. Submit one photocopy of the Student ID Card for the Odd Semester 2021/2022
3. Submit two original receipts of the Final Examination payment (for students of the 2015, 2016, and 2017 cohorts, submit photocopies of the UKT payment receipts with original bank stamps)
4. Submit two photocopies of the high school diploma (STTB)
5. Prepare four copies of 4x6 cm black-and-white studio-printed photos (matte paper) wearing a suit and tie for diploma and graduation transcript (latest photo), and one copy of 3x4 cm photo
6. Submit two photocopies of the thesis title page
7. Submit two photocopies of transcripts from Semester I to VIII
8. Send the soft file of the thesis draft to each Supervisor and Examiner
9. Submit one photocopy of Form C (Thesis Supervisor Assignment Form) signed by Supervisor I & II and Examiner I & II
10. Submit one photocopy of the Study Plan Card (KRP) for Odd Semester 2021/2022
11. Submit one photocopy of the Certificate of Colloquium Presentation Completion
12. Submit one photocopy of the TOEFL certificate with a minimum score of 400
