FORM C

This is the next step after students have been assigned an advisor and have drafted their proposals. Form C is required for the examiners or discussants who will evaluate the students’ final projects.

A. The process for submitting Form C for the thesis is as follows:

1. Students bring the requirements for submitting Form C to the TU of the Mining Engineering Department by carrying: https://bit.ly/PendaftaranFormC

1. Form C Application Form
2. Field Assessment that has been filled in and signed by the Field Supervisor
3. Proof of active student status
4. KRS students in the current semester
5. Transcript of values

2. TU Department provides student application form to KoorProdi
3. Koorprodi provides suggestions for discussants and submits them to the Head of the Department
4. The head of the department through the TU issues Form C and is given to students
5. Students begin guidance
